Step by Step: BOARD RATING APPLICATION, PRC LICENSE APPLICATION, OATH TAKING APPLICATION

BOARD RATING APPLICATION:

1. BRING 1 VALID ID
2. GO TO PRC FIFTH FLOOR (WEAR NO SLIPPERS)
3. GET YOUR RATING.

PRC LICENSE APPLICATION:

1. GO TO PRC
2. GET OATH FORM
3. FILL OUT OATH FORM
4. BRING YOUR CEDULA OR BUY NEW ONE
5. BRING OR BUY LONG BROWN ENVELOPE, LABEL IT WITH YOUR NAME AND PROFESSION
6. HAVE YOUR OATH FORM NOTARIZED (40PHP) AND STAMPED(21PHP)
7. PLACE THE ACCOMPLISHED OATH FORM AND THE CEDULA INSIDE THE ENVELOPE TOGETHER WITH:

• 2 PASSPORT SIZE COLORED PIC, WHITE BACKGROUND AND WITH NAME TAG
• 1 1X1 PIC WHITE BACKGROUND AND WITH NAME TAG

8. SUBMIT THEM AT THE FIFTH FLOOR OF PRC ON THE FOLLOWING SCHED:

• AUG 10: AALA - ANGELES, APRIL
• AUG 11: ANGELES BRIAN - BANTIQUE
• AUG 12: BANTOG - BUNDA
• AUG 13: BUNDALIAN - CAZAR
• AUG 14: CAZCAMO - DANIELES
• AUG 17: DANIPOG - DEVELOS, MA. KRISTINE
• AUG 20: DEVELOS, MONIQUE - FABICULANAN
• AUG 21: FABRE - GAZMIN, HERSHEYS
• AUG 24: GAZMIN RON - JAPZON
• AUG 25: JARA - LIZARDO, JEREMY
• AUG 26: LIZARDO, JINKY - MANINGDING BRIAN
• AUG 27: MANIRANG BRIAN - MOLINA, IRENE

10. GET THE STUD FOR THE CLAIMING OF LICENSE.




OATH TAKING APPLICATION

1. GO TO PHILIPPINE NURSES' ASSOCIATION, FT. BENITEZ ST., MALATE, MANILA STARTING JULY 29.
2. PAY THE FOLLWOING AMOUNTS:

• INITIAL REGISTRATION FEE FOR PNA MEMBERSHIP: PHP 600.00
• TICKETS: PHP 350.00 - INDUCTEE
• PHP 250.00/GUEST

REQUIREMENTS:

(NO UPDATE: JUST BRING THE DOCUMENTS FOR YOUR PRC LICENSE
APPLICATION JUST TO MAKE SURE, AND 1-2 VALID ID'S AND UR NOA AND UR BOARD RATING)



So far, the only thing left for me to do is to return to PRC and submit all the requirements. Also, to request a certificate of passing/board rating or whatever the heck it's called. I hear it's what the hospitals require and not the piece of scrap paper you get after falling in line for hours on the 5th floor of PRC.

After that, a trip to my Alma Mater, Far Eastern University (Institute of Nursing) Morayta, Manila must be scheduled. I still haven't claimed my diploma and paid for the list of additional fees it will manage to extract from me beyond Graduation. This I list below:

• Good Moral Certificate - P101
• Letter of Recommendation - P101
• Transcript of Records - P700 (more or less)
• Alumni Card - P250
• AHSE Certificate - P300
• Diploma - "Free"

Don't get me wrong, I love my school and I'm proud of my batch which has managed to slowly give back the glory of the golden days of FEU-IN. I'm not a fan of the campus administration though. To my institute, I have out most respect and gratitude. I will forever miss the buildings, the faculty and our Dean. To the over-all administrators, stockholders and management of the rest of the money-making venture that is my alma mater, not so much.

I hope you see the difference.
